PENINGKATAN MUTU WAKTU PELAYANAN INSTALASI FARMASI
DENGAN PENDEKATAN LEAN HEALTHCARE
ABSTRAK
Rumah Sakit Umum Daerah (RSUD) Panembahan Senopati merupakan
rumah sakit kelas B yang selalu mengupayakan peningkatan kualitas dengan Indeks
Kepuasan Masyarakat (IKM) sebagai indikatornya. Hasil IKM Tahun 2022
menunjukkan waktu pelayanan memiliki nilai persentase terendah dibandingkan
delapan unsur penilaian lainnya. Instalasi Farmasi adalah salah satu unit dengan
permasalahan waktu pelayanan yang rata-rata waktu tunggunya 43,42 menit, nilai
tersebut lebih besar dari standar waktu pelayanan resep obat jadi (generik) sesuai
ketetapan dalam SK No.129/Menkes/SK/II/2008 yaitu tidak lebih dari 30 menit.
Permasalahan mutu pelayanan pada penelitian ini diselesaikan dengan pendekatan
lean healthcare.
Metode identifikasi lean dalam penelitian ini adalah Value Stream Mapping
(VSM) dengan mengklasifikasikan setiap aktivitas proses pelayanan berdasarkan
kategori VA, NNVA, dan NVA. Selanjutnya dilakukan analisis akar permasalahan
menggunakan Root Cause Analysis (RCA) tools 5 whys. Rancangan perbaikan
terhadap waste kritis diuji dengan model simulasi menggunakan software
ProModel.
Hasil analisis VSM menunjukkan bahwa Persentase Process Cycle
Efficiency (PEC) sebesar 12,15% atau < 30%, yang dapat dikategorikan proses
belum lean. Identifikasi waste mencakup waste extra processing, defect, waiting,
dan motion. Rancangan perbaikan dilakukan terhadap waste waiting yang
merupakan waste kritis pada ukuran batch berkas dan obat dengan tiga skenario
simulasi. Skenario perbaikan terbaik yaitu skenario kedua dengan ukuran batch 10
berkas dan 5 obat dengan hasil penurunan Avg time in system 22,23 menit dari 45,12
menit, dan peningkatan exits rata-rata 158 dari 146. Hasil Future VSM menujukkan
gambaran penurunan waste dan peningkatan nilai PEC sebesar 21,6%.

IMPROVEMENT OF SERVICE QUALITY IN THE PHARMACY
DEPARTMENT THROUGH LEAN HEALTHCARE APPROACH
ABSTRACT
Panembahan Senopati General Hospital is a Class B hospital that always
strives for quality improvement with the Community Satisfaction Index (CSI) as its
indicator. The results of the 2022 CSI show that service time has the lowest
percentage compared to eight other assessment elements. The Pharmacy
Installation is one unit with an issue in service time, with an average waiting time
of 43,42 minutes. This value is greater than the standard service time for dispensing
ready-made medicine (generic) prescriptions as specified in Decree No.
129/Menkes/SK/II/2008, which is no more than 30 minutes. The quality of service
issue in this study is addressed through a lean healthcare approach.
The lean identification method used in this study is Value Stream Mapping
(VSM), classifying each service process activity into VA, NNVA, and NVA
categories. Furthermore, root cause analysis is conducted using the 5 Whys
technique. Improvement designs for critical waste are tested using simulation
modeling with ProModel software.
The VSM analysis results indicate that the Process Cycle Efficiency (PEC)
percentage is 12,15%, which is < 30%, categorizing the process as not yet lean.
Waste identification includes extra processing, defects, waiting, and motion.
Improvement designs are targeted at waiting waste, which is a critical waste in
batch file and medication sizes, using three simulation scenarios. The best
improvement scenario is the second scenario with a batch size of 10 files and 5
medications, resulting in a decrease of Avg time in system to 22,23 minutes from
45,12 minutes and an increase in average exits to 158 from 146. The results of
Future VSM indicate a depiction of waste reduction and a 21.6% increase in PEC
value.
