ABSTRAK
PT. Primissima (Persero) merupakan perusahaan yang bergerak dibidang
industri tekstil dan produk tekstil (TPT) yang terletak di Kabupaten Sleman, Daerah
Istimewa Yogyakarta. Sistem produksi yang dilakukan menggunakan sistem Make
to Order (MTO) sehingga perusahaan melakukan proses produksi setelah
mendapatkan permintaan dari konsumen. Produk yang dihasilkan yaitu kain grey
shuttle dan kain grey Air Jet Loom (AJL). Produk kain grey jenis PS 946 merupakan
salah satu produk yang dihasilkan. Data perusahaan menunjukan bahwa kain grey
jenis PS 946 memiliki tingkat kecacatan yang cukup tinggi. Pada tahun 2023 yaitu
pada bulan Januari hingga Maret dari 45.484 meter yang diproduksi terdapat 6.272
meter mengalami cacat. Berdasarkan permalasalah tersebut, penelitian ini bertujuan
untuk memberikan usulan perbaikan kualitas untuk mengurangi tingkat kecacatan
produk.
Penelitian ini dilakukan dengan metode fuzzy failure mode and effect
analysis dengan bantuan tools pengendalian kualitas yaitu checksheet, histogram,
pareto, peta kendali u dan diagram fishbone digunakan untuk mengidentifikasi dan
menganalisis penyebab kecacatan produk yang terjadi. Dalam memperoleh nilai
fuzzy risk priority number menggunakan perhitungan fuzzyfikasi metode
Tsukamoto.
Berdasarkan diagram pareto diketahui cacat yang paling dominan yaitu
cacat pinggiran jebol dengan presentase cacat kumulatif sebesar 51,2%. Analisis
cacat pinggiran jebol menggunakan diagram fishbone diketahui faktor yang
menjadi penyebab yaitu faktor manusia, mesin dan lingkungan. Faktor manusia
yaitu produk cacat lolos dari pengamatan menjadi skala prioritas perbaikan dengan
nilai FRPN sebesar 375 kategori medium-high. Berdasarkan hal tersebut usulan
perbaikan yang dapat diusulkan adalah operator melakukan proses penyambungan
benang leno dengan benang lusi dan benang pakan secara manual. Usulan tersebut
dapat mengurangi terjadinya cacat pinggiran jebol 10,5 meter/mesin tiap shift.

QUALITY USING FUZZY FAILURE MODE AND EFFECT
ANALYSIS METHOD TO REDUCE
LEVELS PRODUCT DEFECT
ABSTRACT
PT. Primissima (Persero) is a company engaged in the textile and textile
product (TPT) industry which is located in Sleman Regency, Special Region of
Yogyakarta. The production system is carried out using a Make to Order (MTO)
system so that the company carries out the production process after receiving
requests from consumers. The products produced are gray shuttle fabric and Air
Jet Loom (AJL) gray fabric. PS 946 type gray cloth product is one of the products
produced. Company data shows that the gray fabric type PS 946 has a fairly high
level of defects. At the of 2023, namely from January to March, of the 45,484 meters
produced, 6,272 meters were defective. Based on these problems, this study aims to
provide quality improvement suggestions to reduce product defect rates.
This research was conducted using the fuzzy failure mode and effect
analysis method with the help of quality control tools, namely check sheets,
histograms, pareto, u control charts and fishbone diagram are used to identify and
analyze the causes of product defects that occur. In obtaining the fuzzy risk priority
number value using the Tsukamoto fuzzyfication calculation method.
Based on the Pareto diagram, it is known that the most dominant defect is
a broken edge defect with a defect cumulative percentage of 51.2%. Analysis of
broken edge defects using a fishbone diagram is known to be the causal factors,
namely human, machine and environmental factors. The human factor, namely
defective products that have escaped observation, is a priority scale for
improvement with an FRPN value of 375 in the medium-high category. Based on
this, the proposed improvement is for the operator to manually connect the leno
yarn with the warp and weft yarns. This proposal can reduce the occurrence of
broken edge defects of 10.5 meters machine per shift.
