ABSTRACT
PT. Harmak Indonesia, is an Andesite stone mining company located in
Clapar III Hamlet, Hargo Wilis Village, Kokap District, Kulon Progo Regency,
Yogyakarta Special Region Province Andesite Mining PT. Harmak Indonesia,
carried out with an open pit mining system using the quary method. Mining
activities include Unloading, Loading, and Transportation which cause potential
accidents.
In carrying out mining activities, accidents often occur. The emergence of
unsafe working conditions begins with dangerous field conditions and unsafe
work actions and workers ignore safety. From unsafe working conditions and
unsafe work actions, it is necessary to implement a good occupational safety and
health system to minimize various accidents at every stage of Andesite mining
activities.
The percentage of the main causes of accidents during mining activities is
caused by unsafe actions by 70%, unsafe conditions by 30%. Based on accident
data that occurred at PT. Harmak Indonesia in 2020 is calculated using the FR
(frequency rate) formula, with a value of 21.55 and the SR (serverity rate)
formula, with a value of 10.77. After calculating the frequency rate and serverity
rate, work accidents have increased from 2019 to 2020. The increase in frequency
rate and serverity rate in 2020 is due to unsafe actions by workers who are not
aware of personal protective equipment, so prevention efforts by means of a
human approach in order to minimize work accidents in the coming year.
